REGISTRATION DEADLINE IS SUNDAY, NOVEMBER 29, 2009 To Secure a spot and register your team(s) now for the 2009-2010 season please DOWNLOAD THE REGISTRATION FORM, complete as much information as you have for now, and submit it electronically to the Jersey Premier Futsal Registrar with payment by PayPal (see button below), or send by mail to: Jersey Premier Futsal 356 Old York Road Flemington, NJ 08822 Team Registration fee is $595/team which includes 8 USFF Futsal passes for each team. Each additional player pass is $7.00/ea., please include this sum in your registration. You may pay by using the PayPal button below. You will be able to pay using Visa, Master Card, Discover, or AMEX. PayPal is a secure and easy way to pay for your team's entry. Registration is not completed until payment is received. After you downloaded the registration form and registered your team with Jersey Premier Futsal, coaches must create teams and register players with the United States Futsal Federation by following these easy instructions. Coaches may form more than one team. This must be done before the in-person registration to be held Saturday, December 12 from 2:00 PM to 5:00 PM at the Hunterdon County Library located on Route 12, Raritan Tp., NJ Use the PayPal Button Pay $595/team, and $7.00 for additional player passes. |
